Here are a few things to know about the vaccine appointment:

  • Anyone under 18 must be accompanied by an adult
  • If under 18, a parent or guardian must either accompany the child, or
  • If a parent or guardian cannot attend, they must complete THIS FORM and send it with the adult brining the child.
