What is Proxy Access?
MyChart allows patients to access their healthcare wherever they are whenever they need it. Proxy access allows parents, legal guardians, caregivers, and others to log into their personal MyChart account, and then connect to medical information for the patient they represent. Available access is based on the age of the patient and proper authorization.
There are several ways to get access to another patient’s MyChart account depending on the patient’s age. Active MyChart users can assign and/or revoke proxy access directly through MyChart. Go to Menu > Share My Record > Friends and family access > Invite Someone. If the patient is unable to assign proxy access through MyChart, proxy access may be requested by completing our Proxy Access Forms below.
- Dependent Children Aged 11 and Under: Typically, the mother is automatically assigned as the newborn’s MyChart Proxy. If a second parent or legal guardian wants MyChart Proxy access, they need to request access with the Child Proxy Form.
- Dependent Children Aged 12-17: Adolescent patients in this age group will be able to grant or remove MyChart Proxy access directly in their MyChart. Parents or legal guardians may request access through the Child Proxy Form. MyChart and Proxy access is limited for the 12 to 17 year old age group. See more here.
- Patients Aged 18 and Older: Any patients 18 and older can grant or remove MyChart Proxy access directly through MyChart. People who wish to have MyChart Proxy access can also submit the Adult Proxy Form.
