NOAH offers enrollment assistance to individuals and families in need of health and community resources. Our community resource specialists assist with enrollment of various programs and services like Medicaid and Medicare.

Program Enrollment Assistance
NOAH is dedicated to providing you with a full range of quality primary care services at an affordable price, regardless of your ability to pay. If you are uninsured or under-insured, you may qualify for programs and services with reduced fees based on income and family size.
AHCCCS Medical Assistance
AHCCCS is Arizona’s Medicaid agency which offers several health care programs to serve Arizona residents. Programs are based on meeting certain income and other requirements but in general, you are likely to qualify for AHCCCS if:
- Your family includes children under age 19
- Your family income meets the Arizona State guidelines. Our specialists can tell you more or visit the AHCCCS website for specific information
- You have been a resident of Arizona for 5 years or more

Discount Program
NOAH accepts all patients regardless of their insurance or financial status. If you are eligible, our discount program or Sliding Fee Schedule (SFS) allows you to pay reduced rates for services based on specific qualifications, including family income, size and situation.SNAP (Formerly Food Stamps)

Here are the requirements for Medicaid, TANF, SNAP, KIDSCARE:
- Consent Signature: English or Español.
- Application signature: English or Español.
- Income from last 30 days
- Have Full Names, Social Security Numbers, AND Date of Births– For everyone in the Household ready for a Virtual Phone Call from one of our NOAH’S Community Resource Specialists.
- For SNAP – Have your utility bill, rental or mortgage paid, and phone bill ready for a Virtual Phone Call from one of our NOAH’S Community Resource Specialists.
