Neighborhood Outreach Access to Health’s (NOAH) mission is to provide compassionate, evidence-based quality healthcare for our community. We are transforming the health of our community by providing accessible and affordable healthcare while improving health outcomes.
As a 501(c)(3) not-for-profit organization, we offer an approach to health that treats the whole person. We focus on working with patients to get well while respecting their diverse cultures and needs. Our community health centers combine family medicine, pediatric care, dental services, behavioral health and nutrition services with a team of experienced professionals who are committed to high-quality care.
NOAH accepts most insurances, including AHCCCS. NOAH also offers a discount program for those who qualify.
Getting started as a NOAH patient is easy!
Step 1- Locate a NOAH Health Center Near You
NOAH provides comprehensive healthcare services at locations across Scottsdale, Phoenix and Glendale. Find the location nearest you.
Step 2 – Find a Provider that Fits You
Our team of health professionals always puts our patients first and provides quality care across a wide range of services.
Step 3 – Schedule Your Appointment
Once you’ve located your center and chosen the health care provider you’d like to see, contact us at 480-882-4545 to schedule an appointment. Most of our centers offer extended hours for early and evening appointments.
Our Patient Engagement Representative will ask you basic questions to set up your record including name, address, date of birth and your insurance plan. See a full list of insurance plans NOAH accepts.

Step 4 – Make the Most of Your Appointment
