Our records indicate you currently receive discounted NOAH services and/or medication through our Sliding Fee Program.
Your Sliding Fee (Discount) Program enrollment has either expired or is about to expire. To continue receiving discounted services and prescription medication, you must re-apply. Request an appointment with Community Resources online or call 480-882-4545.
Effective January 1, 2025, you must use NOAH’s Pharmacy to continue receiving discounted prescription medications. Walgreens will no longer fill your prescriptions at the discounted rate.
