Neighborhood Outreach Access to Health (NOAH) conducts a Community Health Needs Assessment (CHNA) once every three years to ensure health center services and locations align with the needs of the neighborhoods we serve. This exercise helps us to identify priorities for growth and change and provides a benchmark for understanding NOAH’s impact on the overall health of our community.
