By entering your contact information, you are consenting to the Official Rules of this sweepstakes. Entry must be submitted in person at NOAH Cholla Health Center, 8705 E. McDowell Road, Scottsdale, Arizona, 85257 during the sweepstakes period beginning Saturday, March 2, 2024 at 9 a.m. MST and ending Saturday, March 2, 2024 at 3 p.m. MST. Valid entries must have four colored stamps representing participation in booths or activities in each of the colored event “zones”. Number of eligible entries received determines odds of winning.
One (1) winner will be selected at random and notified by telephone and/or email Monday, March 4, 2024. We will attempt to notify the winner three (3) times over the course of seven (7) days ending at 12 a.m. MST, Monday, March 11, 2024. If prize has not been acknowledged by the winner, an alternate winner will be selected. Arrangements for prize pick-up will be made upon winner notification. Winner’s failure to claim prize within seven (7) days of set pick-up date will result in selection of an alternate winner. By accepting prize, winner agrees to the use of their name and likeness in printed and digital NOAH publications including but not limited to social media, newsletters, and annual reports. By participating in this sweepstakes, you agree to release and hold harmless NOAH from and against any claim or cause of action arising out of participation in the sweepstakes or receipt or use of any prize.
Sweepstakes winner will receive a gift certificate valid for a two-night (2) stay at Holiday Inn Express and Suites Old Town Scottsdale located at 3131 N. Scottsdale Rd., Scottsdale, Arizona 85251. All rooms are spacious suites, which have either one kind bed or two queen beds, pull-out sofa sleepers, two (2) HDTVs, pay per view movies, microwave, mini-fridge, and desk. The hotel offers shuttle, complimentary breakfast, free wired and wireless internet, outdoor heated pool and spa, fitness center, and business center. Must reference certificate when securing reservation. Reservation must be made directly with hotel’s general manager who can be reached at 480.675.7665. Must surrender certificate upon check-in. Valid credit card required for incidental charges. Reservations based on availability. Not valid during the month of March. Certificate expires December 31, 2024.
